Description
This is an informal survey of participants at the March 22, 2015 teacher workshop in Ludlow, MA.

- Enter your Commute Time to and from work to this workshop in integer minutes.
- Enter your Commute Distance to this workshop in decimal miles.
- Enter your years in your current job.
- Select your animal preference.
- For the Location field, use http://itouchmap.com/latlong.html to pick your favorite vacation place. Then copy the latitude and longitude coordinates into the data set (remember to pay attention to negative numbers).
Then, click Save. (Don't click Add Row).
